Issue:
Whether or not to approve payment for Hearing Aids for LEOFF member, and if approved, for what amount.

Background:
The board must decide whether or not to approve the request for hearing aids in the amount of $1,699.99 or $3,305.14. The member prefers hearing aids from Ascent Avada. He has submitted estimates from:
Costco KS 7.0 RIC Digital $1,699.99
Avada Bernafon Juna 9 Nano Rite $3,304.14
The member is requesting hearing aids in accordance with LEOFF I policies and procedures. The member submitted quotes from at least two licensed audiologists. One quote is from Costco. He has a current hearing test and a referral from a licensed hearing aid examiner. The hearing aids have three-year warranties. The member has not requested hearing aids in the last three years. He last requested hearing aids in 2011.
