Whether to approve payment for Intense Pulsed Light Therapy.

Background:
Retired LEOFF 1 Member is requesting approval of Intense Pulsed Light Therapy in the amount of $1,400 for extreme dry eye. The therapy requested has a success rate of 80 - 90%. Per the health care provider section of the Application for Payment of Services, a dry eye mask and been tried and failed. An alternate treatment is Miebo medicated drops and notes this would be ongoing treatment and cost. I inquired with the members insurance carrier and Miebo is a covered medication on the members plan. I sent an inquiry to the member to see if Miebo has been tried, however I did not receive a response from the member.
