Approve or deny payment for hearing aids for LEOFF 1 member. If approved, for what amount.

Background:
The board must decide whether or not to approve the request for Case #01-18, hearing aids in the amount of $5,050.00, or $2,599.98. He has submitted estimates from:
Costco Resound Forte Ric $2,599.98
Avada OPN 2 Mini Rite $3,304.14
The member is requesting hearing aids from Avada. In accordance with LEOFF I policies and procedures, the member submitted quotes from at least two licensed audiologists, and one is from Costco. He has a current hearing test and a referral from a licensed hearing aid examiner. The hearing aids both include three-year warranties. The member has not requested hearing aids in the last three years.
