Background:
The board must decide whether or not to approve the request for hearing aids in the amount of $3,800.00, $5,080.00, or $2,499.99. He has submitted estimates from:
Miracle Ear 5500 RIC-BTE $3,800.00
Hearing Healthcare Center Inc. Siemens/Signia Pure 312 5nx $5,080.00
Costco Resound Forte RIC Hearing $2,499.99
The member is requesting hearing aids from Miracle Ear. In accordance with LEOFF I policies and procedures, the member submitted quotes from at least two licensed audiologists. He has a current hearing test and a referral from a licensed hearing aid examiner. The hearing aids all include three-year warranties. The member has not requested hearing aids in the last three years.
