Background:
The member recently underwent a neck surgery and continues to experience on-going neck and back issues. His healthcare provider has prescribed a lift chair, and the total purchase for the chair is $2,937.81. The lift mechanism is covered by insurance in the amount of $326.19. The member is seeking payment for the remaining balance of $2,611.62. The member has submitted the request in advance and would like to purchase the chair from an authorized Durable Medical Equipment (DME) provider in accordance with the LEOFF I policies and procedures. The board must decide whether or not to approve payment in the amount of $2,611.62.
