Issue:

Whether to approve payment for hearing aids for LEOFF 1 member and if approved, for what amount.

Background:

The board must decide whether or not to approve the request for hearing aids in the amount of $3,726.27 or $2,499.98.  He has submitted estimates from:

 

Costco                                                               Resound RIC                                                               $2,499.98

Avada Hearing Center                     V5261 Oticon Opn 1 miniRite                     $3,726.27

 

The member is requesting hearing aids from Avada.  In accordance with LEOFF2 policies and procedures, the member submitted quotes for at least two licensed audiologists.  The hearing aids include a three year warranty.  The member has not requested hearing aids in the last three years.
