Retired LEOFF 1 member is requesting approval for a dental extraction and placement of implant and crown in the amount of $5,534.00. This member is covered by Regence dental insurance as provided by the City and the requested amount will be reduced by any amount paid by insurance. This request in in accordance with the LEOFF 1 policies and procedures.
